Mutanus the Devourer is spawned by an event. To start this event you must first kill all 4 Fanglords then go back to the start of Wailing Caverns and speak with Disciple of Naralex. You will have to successful escort Disciple of Naralex to a chamber where a series of events happen described in the above post.
