HumanaVitality, to be renamed Go365 by Humana effective January 1, 2017, is a wellness and rewards program deeply rooted in behavioral economics and actuarial science to motivate and reward members for taking steps to improve and continue their healthy behaviors.

How do I get my Go365 points?
Sign in to the Health IQ app or website and complete a quiz on a variety of health topics. Connect your Go365 account to Health IQ then set the activity within the Go365 App to automatically earn your Points. Start a new fitness habit and submit photo proof to earn your Points.
How much does Go365 cost?
The Bucks average payment of $0.51 – $3.65 PMPM is an estimation. Actual rewards payment will be calculated monthly based on actual results and outstanding balance is available monthly on billing statements.
Can I use my Humana healthy food card at Kroger?
With the Healthy Foods Card, Dual-Eligible (Medicare/Medicaid) members can purchase approved food and beverages at a variety of national chains, such as Walmart, Kroger (including their brand portfolio), Walgreens and CVS.
